Cal Poly Humboldt students and employees may report violations of CSU’s Nondiscrimination policy by filing an “Incident Report” found at https://link.humboldt.edu/TitleIXReport  Students and employees have the right to file a formal complaint, found at https://link.humboldt.edu/TitleIXFormalComplaint, which requires their signature to initiate the formal complaint process.
