Common Report Forms
Listed below are direct links to the most frequently used reporting forms.
If you are unsure which form to use:
- Choose the most appropriate tab above to access a dropdown list of issues/concerns. Each option will have more information about how to report the complaint if you wish to take action to adress the matter.
- If you are still unsure about which report to use, you may submit the "Other Conduct of Concern" report. You will be contacted by the Complaint Process Coordinator, who will then direct you to the appropriate person to address the matter.
Basic steps and forms to report academic dishonestly are as follows:
- Faculty should notify the student, first, and meet to discuss the alleged infraction. At that meeting, the faculty will document the details using the Academic Dishonesty Referral Form and have the student sign the form.
- Faculty will then submit an Academic Dishonesty Reporting Form to the Office of Student Rights and Responsibilities and attach a pdf of the signed Referral Form.
CARE Student of Concern Referral (may be submitted by staff, faculty, and off-campus individuals)
CARE Self-Referral Report (may be submitted by currently enrolled students)

Ombuds Office
Humboldt Ombuds is a free service, available to Cal Poly Humboldt students, faculty, and staff. They offer a safe place to discuss problems or issues within the University and can help identify options for addressing specific concerns and resolving conflicts. Ombuds treat all inquiries as confidential (except as required by law or where there appears to be imminent risk of serious harm). They do not take a side in disputes. Overall, Ombuds are advocates for fairness and equity.
