Reporting Guide for Addressing Issues of Concern and/or Filing Complaints

Statement of Purpose

Cal Poly Humboldt is committed to fostering and maintaining a safe, inclusive, and professional environment for all students, faculty, staff, and visitors. We are dedicated to building a culture of fairness and accountability where every member of our campus community can learn, work, and grow in a supportive and respectful setting. 

We recognize the importance of providing clear avenues for individuals to share concerns about conduct that is affecting their well-being or the campus climate. To support this, we have created this website which provides concerns & action guides and reporting options to notify the University when unprofessional conduct by a member of the campus community has impacted an individual's ability to concentrate on their academic or professional responsibilities. 

Our goal is to create a community where serious concerns can be reported and addressed appropriately – free from fear of retaliation. What you have experienced matters; you deserve to be heard and have your concerns addressed and securely documented.

Best first steps to take when dealing with unresolved conflict

• Speak directly with the individual involved, if safe and appropriate.
• Contact a supervisor or administrator if direct communication is unsuccessful or not feasible.
• Consider utilizing the University Ombuds
• Submit an OCC report if informal efforts have been exhausted or additional support is needed.

What form should I use?

• The "Other Conduct of Concern" Incident Report specifically designed for situations that do not fall under the established laws or policies. This form may be used in these instances or if you are uncertain about which type of report is appropriate.
• Our primary goal is to ensure that the University is informed about any concerning behavior. It is preferable to report something that may seem insignificant rather than risk overlooking potentially harmful behavior. 
• You do not need to be worried about submitting the incorrect form. Each report is thoroughly reviewed, and if necessary, it will be forwarded to the appropriate office for processing.
• All reports are processed within 1-2 days of being submitted. Individuals submitting a report can expect their complaint to be dealt with promptly and fairly and in accordance with the appropriate procedure.
