• Donation Deposit Form  
  Use this form for processing financial donations
• In-Kind Gift & Property Proposal Form  
  Use this form for any donated physical items (i.e. supplies, art, vehicles)
• Model Release Form  
  Release form authorizing Humboldt to use photos/videos
• Request for Raiser's Edge Access  
  Use this form to add a new user to the Raiser’s Edge platform, the campus’ fundraising database
