Extenuating Circumstances and Documentation Examples

Mental Health Challenges: Severe anxiety, depression, PTSD, or other mental health conditions impacting academic performance or attendance

Example of Documentation:

  • Letter from a licensed mental health professional (e.g., counselor, therapist, or psychiatrist) detailing the condition and its impact.
  • Discharge paperwork from a psychiatric facility (if applicable).
  • Treatment plan or ongoing therapy documentation.

Medical Issues: Chronic illness, injury, or hospitalization.

Examples of Documentation:

  • Medical note from a physician or hospital outlining the condition and duration of care.
  • Discharge summary from hospital visits.

Housing Insecurity: Eviction, homelessness, or unsafe living conditions.
