Welcome!

Our intention is for the CARE Services Program to be a safe and trusted campus resource that supports students who are overcoming extenuating circumstances. This site contains information to assist you in accessing on and off campus resources related to basic needs and health and wellbeing services. Click the drop down menus (on the right) to learn more about resources in our community. We know that sometimes it can be difficult to access resources on your own, and we are happy to help students navigate the process to make it more accessible.

If you have any questions, or if our services sound like they could be helpful, please contact our office at (707) 826-3504 or email us at dos@humboldt.edu. The CARE Services office is located in the Dean of Students Office in Siemens Hall 211.

Connecting with CARE Services:

Our services are currently available by appointment to matriculated Cal Poly Humboldt Students in-person on campus, virtually through Zoom, and phone.  At this time we are not able to accept same day drop-in appointments. Below are our current office hours:

Monday - Friday: 9:00am - 4:00pm

Students:

Students can schedule an appointment by emailing dos@humboldt.edu and we can also be reached at our main phone line (707) 826-3504 where you may leave a voice message for someone to return your call.  Students may also submit a CARE self-referral form and someone from CARE Services will reach out to offer a meeting.

Staff and Faculty:

Staff and Faculty submitting a referral for a student of concern, please use the CARE referral form as we will prioritize these referrals.  If you have questions about referring students or would like to consult with CARE Services, please email dos@humboldt.edu or call (707) 826-3504.

Community Members:

Community members submitting a referral for a student of concern, please use the CARE referral form as we will prioritize these referrals.  If you have questions about referring students or would like to consult with CARE Services, please email dos@humboldt.edu or call (707) 826-3504.

Maxient Correspondence:

CARE Services will communicate with students through Maxient — a secure, web-based case management application that is used to maintain student interactions and information with CARE Services.  Students may access this information by request.

If a student is referred to CARE Services or is working with CARE Services, they may receive an email from “notifications[at]maxient.com.”  The email will show the sender from a humboldt.edu email address, with their name followed by “(via Maxient).”

It is important to know that this is an authentic email from the university. It is not spam or a phishing attempt.

In addition to a brief explanation, the body of the email will typically contain a link to a letter. The letter is accessible through a login page. Once logged in to the Maxient portal, students can access their letter, which will be a .pdf document. Student may print or download and save the letter for their own records.

If you have difficulty accessing a letter in Maxient — or want to confirm the legitimacy of the email — contact us at dos@humboldt.edu or (707) 826-3504.
