RSO Forms

Forms for starting and maintaining RSO Processes:

1. Intent to Organize - Use this form to start a new Registered Student Organization. If you are unsure if an RSO has existed in the past, please email us at clubs@humboldt.edu
2. Organization Registration - Use this form to transition your RSO to a new school year or restart an RSO that has been on hiatus
3. Jackpot Funding Request - Use this form to apply for RSO funding from the Jackpot
4. Request for Authorization to Travel (RSO) - Use this form to request to travel as an RSO
5. RSO Traveler Information Form - This form will be filled out by INDIVIDUAL travelers from RSOs, every traveler is required to fill one out
6. RSO Membership Agreement Form - Fill this form out to be added to a specific RSO as a member
7. RSO Fundraising Request Form - Fill this out to request to fundraise for your RSO account
8. Tabling, Chalking, Staking, and Poster Form - Fill this out to request tabling dates, post flyers, request chalking or staking to promote RSO or events on campus
