Survey Support

The IRP office will be the repository for all surveys administered to students campus wide. We are available to assist in the creation, deployment and analysis of all surveys. We have the software and staff to help you create a survey and the techniques to assure the best response rates possible. After reviewing the table below please fill out a Survey Request Form and we'll set up an appointment to meet with you and go over your survey needs.

Do I need Authorization?

Please review the table below to determine if you require authorization from IRP for your survey.

Step 1:

Is the survey for academic research?

Yes →

No – Proceed to Step 2

Obtain Institutional Review
Board approval, if relevant. Visit www.humboldt.edu/hsuf/irb.php for information and forms for approval or contact irb@humboldt.edu. Then proceed to Step 2 below.

Step 2:

Are you planning to survey a subset of the HSU population (students, faculty, or staff)?

Yes →

No – Proceed to Step 3

Complete the Survey Request Form to obtain authorization, support and administration of your survey from the Office of Institutional Research and Planning. This ensures that individual or unit designed surveys do not conflict with institutionally sponsored projects and prevents oversampling of a population.

Step 3: (only if Step 2 does not apply)

You do not need to obtain authorization from the Office of Institutional Research and Planning. Some free survey tools and resources are listed below for your convenience.
