Diversity Programs Funding Grant Evaluation Form

All individuals receiving Diversity Program Funding Grants are required to submit, upon completion of the event, an evaluation of the funded event. These evaluations are critical to our ability to assess the impact of these grants and to continue to advocate for this important funding mechanism.

We suggest that you and any other coordinators of your program meet after the event to debrief and discuss your thoughts about the overall success of the program. After doing so, please fill out and submit to the Diversity Program Funding Committee this online evaluation form.
